Diabetes Self-Management Education Program

If you're one of the 17 million Americans dealing with diabetes, it's worth making sure you have the best care and support available. We'll teach you self-management techniques and help you and your loved ones better understand your condition. Our education program is recognized by The American Diabetes Association as a center of excellence for diabetes education. And our team of certified diabetes educators, registered dietitians and advanced practice and registered nurses work with your primary physician or endocrinologist to help you:

  • Plan meals
  • Monitor blood glucose levels
  • Self-administer medication
  • Handle emergencies
  • Prevent complications
  • Implement healthy lifestyle habits

Risk Factors and Symptoms of Diabetes

Of the 17 million people that have diabetes, 5.9 million don't even know they have the disease— you may be one. If any of the following factors apply to you, you may be at risk for developing diabetes:
  • Being overweight
  • Living a sedentary lifestyle
  • Having a blood relative with diabetes
  • Being 40 years of age or older
  • Being of African, Hispanic or Native American descent
  • Having a history of gestational diabetes
  • Having a baby weighing more than nine pounds at birth
  • Having a history of hypertension
  • Being over-stressed or having had a recent surgery or illness

Also check for the following warning signs:

  • Excessive thirst
  • Frequent urination
  • Fatigue
  • Blurred vision
  • Constant hunger
  • Numbness and tingling in feet
  • Frequent infections
  • Slow healing process
  • Weight loss
